How to create a Self-Assessment in BlockSurvey

Self-assessments are useful for evaluating knowledge, skills, or personal attributes based on predefined criteria. This guide walks you through setting up a self-assessment that calculates scores and provides personalized feedback on the Thank You screen in BlockSurvey.

Example Scenario

Suppose you're creating a self-assessment to evaluate anxiety. In this case, we’ve used a pre-existing template available in BlockSurvey.

Sample Question: Feeling nervous, anxious, or on edge? (Single choice)

  • Not at all (0 points)

  • Several Days (1 point)

  • More than half the days (2 points)

  • Nearly every day (3 points)

Similarly, you can add more questions with assigned scores to determine the final assessment result.

Steps to set up

  1. Go to the Design screen of the survey and add your questions and answer choices.

  2. Click Add Weight/Score and assign points to each option. Repeat for all questions.

  3. All numeric scores are summed and stored under the Score variable by default.

  4. If you need custom calculations, go to Settings -> Assignment & Formulas -> Numeric Expressions and define your expressions.

  5. Navigate to the Outcome tab on the Thank You screen.

  6. In the Summary section, click Add Summary to define conditions for displaying different feedback based on the total score.

  7. You can add multiple summaries as well as set up multiple conditions for a summary.

  8. Once everything is set up, click Publish and test it.

How does it work for the respondent?

When respondents submit their answers, BlockSurvey calculates their score, evaluates the assigned conditions, and displays the corresponding feedback on the Thank You screen.
